Completing the evidence collection checklist when you are on-site will help ensure you have gathered all of the required evidence.

Ten (10) points of eligable ongoing professional development activities must be detailed in the Continual Professional Development log (which is available for download on our website), together with evidence of completion for each CPD activity. This only needs to be completed once per year of certification - upload the same document for both JAF forms.

You must provide a Certificate of Electrical Safety (or equivalent) or electrical risk assessment. If a person other than a licensed electrician or electrical inspector conducted the electrical risk assessment, you must provide evidence of their competency to complete the assessment. A competent person to conduct the assessment may have a specific definition in your jurisdiction.

Details of the trainer, team supervisor or co-installer on-site on this job are required. This person may be contacted as part of the assessment process. If they are a Certified Insulation Installer you must include their Certification Number, which can be found here.
